Adult Family Care

The Arc of Opportunity’s Adult Foster Care (AFC) Program helps support a safe, nurturing home for people who need assistance to complete the activities of daily living, such as dressing, bathing, eating nutritious meals, walking, and mobility.

AFC is a MassHealth-funded program that provides in-home support to care providers, family members and AFC members.

Monthly visits from the Registered Nurse and Care Manager offer detailed health service coordination, communication and related support. This dynamic service delivery plan allows for AFC member engagement in the planning and receiving of care in the comfort of their own home.

Through AFC, people become Care Providers for family members or others with any medical diagnosis. These could be intellectual, developmental or physical disabilities that result in the member needing assistance and oversight to safely manage specific needs while living at home.

The AFC program provides a tax-free stipend to the Primary Care Provider. AFC Care Providers are thoroughly pre-screened to become qualified candidates. The AFC member and Care Provider live together in a family household. Households must pass inspection in order to be qualified. The Staff of The Arc is here to assist AFC members and Care Providers with accessing program services. These services include:

  • Assisting existing families through the process of becoming qualified
  • Care Provider pre-screening
  • Clinical and social member assessment
  • Nursing and care management support
  • Individualized training
  • Plan of Care to identify all goals and objectives to maximize care
  • Monthly in-home visits
  • Information about community resources and other local programs
  • Assistance with identifying alternate caregivers to help when your primary caregiver is not available
  • 24 hour on-call system
